- 2 copies of Borang 'G' to be fill, please attached 2 copies of passport size color photo.
- No. 11-Negeri Domisil – to be fill by foreigner
- No Instant Photo

- Please fill in all the particulars in Borang 'T'
|
1.3 |
- Please fill in the present address and need not follow the NRIC's address.
|
1.4 |
- Under age of 21, father or guardians need to sign the Borang N in front of assistant Registrars of Marriages.

- For those lady applicant ages between 16 to 18, need to obtained approval on Borang 'E'.

1.6 |
- Borang 'G' No.12 Please fill in your profession or Job title.
|
1.7 |
- Applicant without religion will be rejected.
|
1.8 |
- The said Assistant Registrar of Marriages in unable to register for those who had registered oversea or had carried out traditional wedding before 1/3/1982 or is a Muslim.

- All documents should be kept clean and tidy.
|
2. Taking of Oath
2.1 |
- After filled in the forms, the couples have to take oath in front of Commissioner for Oath.
- Please bring a long both copies of Borang 'G' and your original NRIC.
- Both parties have to be present.

3. Return Forms & Appointment
3.1 |
- Submit Borang 'G' and Borang 'T'
|
3.2 |
- Attached 2 Photostat copies of NRIC and Birth Certificate of each applicant.
|
3.3 |
- Attached 2 Photostat copies of copies of two witness's NRIC each.
|
3.4 |
- For divorcee, please attach 2 Photostat copies of Divorce Certificate.
- For widow/widower, please attach 2 Photostat copies of Death Certificate.

3.5 |
- For foreigner, please attach original copy and one Photostat copy of Single Status Letter, 2 Photostats copies of passport and supporting documents.

3.6 |
- All NRIC (front & back) to be Photostat on same page in A4 paper.

3.7 |
- All copies should be neat and clear, changing of appointment date and time is not encourage.
|
3.8 |
- Appointment shall be made 7 days ahead.

4. On the Registration Day
4.1 |
Be punctual & Bring along all the original documents. |
4.2 |
Attire should be neat, no T-shirt, short or singlet. |
4.3 |
Marriage certificate will be issued once registration ceremony is over |
